Alibi BJJ Academy General Assumption of Risk Form
1. Introduction
Martial arts training at Alibi BJJ Academy involves physical activity and self-defence techniques that inherently carry a risk of injury. This form ensures that all participants (or their parents/guardians if under 18) understand and acknowledge the risks involved before engaging in training.
2. Acknowledgement of Risk
By signing this form, I acknowledge and understand that:
- Participation in martial arts involves inherent risks, including but not limited to bruises, sprains, strains, fractures, concussions, dislocations, and other potential injuries.
- More severe injuries, while rare, can occur, including head injuries, nerve damage, or serious trauma.
- The nature of martial arts training involves close contact with instructors, training partners, and equipment, which increases the risk of accidental injury.
- There is a possibility of experiencing emotional or psychological stress during training.
- There is a risk of exposure to communicable diseases (such as colds, flu, or COVID-19), particularly in a training environment where physical contact is involved.
3. Voluntary Participation
I confirm that:
- My participation (or my child's participation if signing as a parent/guardian) is voluntary and I am aware of the risks involved.
- I have had the opportunity to ask questions regarding the nature and risks of training at Alibi BJJ Academy .
- I understand that I (or my child) have the right to stop participation at any time if uncomfortable with any aspect of training.
4. Safety and Responsibility
I acknowledge that:
- The instructors and staff at Alibi BJJ Academy will take all reasonable precautions to ensure safety, but they cannot eliminate all risks associated with martial arts training.
- I (or my child) must follow all instructions, safety rules, and guidelines given by the instructors.
- I am responsible for ensuring that I (or my child) wear appropriate protective equipment when required and that I report any injuries or medical concerns to the instructor immediately.
5. Medical Conditions
I confirm that:
- I (or my child) am physically fit to participate in martial arts and have no known medical conditions that would pose a risk to myself, instructors, or others.
- If I (or my child) have any pre-existing medical conditions (such as asthma, epilepsy, heart conditions, or prior injuries), I have disclosed them to the instructor before training.
- I understand that it is my responsibility to consult with a medical professional if I have any concerns about my (or my child’s) ability to participate.
6. Waiver and Release of Liability
By signing this form, I agree to the following:
- I release and hold harmless Alibi BJJ Academy , its instructors, staff, volunteers, and any associated parties from any and all liability for personal injury, accidents, or damages that may arise from participation in training.
- I understand that this release applies to any injury or loss that may occur due to negligence (except where prohibited by law) and includes injuries arising from the use of facilities, training methods, or the actions of instructors and training partners.
- This waiver does not affect my legal rights in the event of proven gross negligence or intentional misconduct by the club.
7. Emergency Contact and Consent
If signing on behalf of a child, I confirm that:
- I am the legal parent/guardian of the named participant.
- I give consent for my child to participate in martial arts training at Alibi BJJ Academy .
- In case of emergency, I give permission for the instructors to administer basic first aid or seek medical assistance as necessary.
8. Confirmation and Agreement
I confirm that:
- I have read and understood this Assumption of Risk Form.
- I am signing this agreement voluntarily and with full knowledge of its contents.
- I agree to abide by the safety rules, policies, and procedures of Alibi BJJ Academy .
